I think some of the main problems of the song are:
1.) The tone she chose to sing it in
2.) The fact that she keeps repeating "Perfect Illusion" over and over again (kinda like in "Applause" which is a lazy way to get it stuck in your head).
3.) A weak chorus, the bridge is stunning though!
4.) A monorhyme scheme in the verses, where each ending line rhymes with the next.
5.) And finally the massive hype didn't help from Mark and Gaga. People put way to much expectation on it, there was no way it could have lived up :(

BTW I'm a massive Gaga fan (if you couldn't tell by my username). I mean no disrespect I'm still going to support her and buy it on iTunes. Maybe a couple more listens will help.

I hate it when people say that they want the old Lady Gaga back. Of course, we all fell in love with the girl who wore a meat dress at the VMA's, came out of an egg at the Grammys, swallowed rosary beads in Alejandro, offended the Roman Catholic Church, and made the best dance-pop songs of the 2010s, but the true fans or Little Monsters are the ones who actually appreciate Gaga's work, no matter if she changes from dance-pop, to heavy metal, to dubstep and EDM, to jazz, and to rock and country. I have been a fan of Gaga ever since June 2015. I wish I was a fan of her sooner, but I was so wrapped up in wanting to be like everyone else, so I listened to their type of music (hip-hop and rap), but I realized that I need to listen and put myself under the influence of someone who is very eccentric and weird, and who supports the LGBT community and doesn't care about your race, religion, sexuality, etc... She just cares about your personality, and this song, along with Million Reasons, A-YO, and her other songs on her heavily awaited album that will be released tomorrow, reflects what she has been wanting to release for years. This is a very personal album for her, dropping all of her personas to go with the subdued version of herself, showing the raw and unedited version of who Stefani Joanne Angelina Germanotta really is. I know that Perfect Illusion was a commercial flop, but she provided one of the most intense and emotional vocal performances that I have heard so far, and that is what I, and many of you all, should appreciate about her. She has a versatile voice. She can go from singing a mid-tempo dance song to a slow ballad with huge vocal strength. Lately, she has been so underappreciated, but who can say that they were able to perform at 2 Academy Awards back to back, perform at the Super Bowl for the national anthem and will perform again for the halftime show, get nominated for a Grammy, an Oscar, and a Golden Globe in the same year, win a Golden Globe, has time to show up on American Horror Story, one of the biggest shows on television, and can still drop an album for all of our ears to enjoy and our mouths to drop at such talent. I am pretty sure barely anyone, if even anyone, can say that. So think about this before you are so quick to trash Gaga's masterpiece that she has wonderfully created, and I can't wait to hear your album, Mother Monster. Why is everyone so negative? Nothing wrong with changing musical direction every now and then! Did you really expect Gaga to stay and have the same sound as in 2010 just because that's the style you like?A true artist should evolve and change with time, not do the same thing over and over. This may not be her best song but at least it's better than any of the songs in ARTPOP.
